Social support and optimal health care are key to well-being for people experiencing chronic diseases, living with dementia (PLWD) and caregivers. Many informal caregivers (family) report higher stress levels when balancing work and family life, including being unpaid or over burdened. Finally, COVID-19 caused unforeseen shut-down or radical re-shaping of services, causing elongated self-isolation, depriving timely & efficient care.
LocateMotion aims to track progress of dementia during cognitive decline by capturing vitals, daily notes, medication effects, feelings, behavior and finally assessing and suggesting virtual interventions based on risk-levels.
Offering 24/7 virtual interventions and data collection, will increase patient and caregiver engagement and knowledge about dementia (during decline stage) and related modifiable factors, improving healthy living behaviours and potentially delaying dementia progression in high risk populations.
Additionally, SenSights.AI offers virtual community navigation support for PLWD, their family and caregivers, including facilitation and promoting linkages among community service providers. Finally, measuring vitals like blood pressure (BP), oxygen or glucose levels, body weight and body fat, will help dementia patients especially caregivers manage their care virtually, providing doctors with current, accurate and vital data, to support treatment decisions and quality of life.
This project considers intersection of technology utilization, enhancement of existing practices and inclusion of global best practices for all aspects of care for patients, families and caregivers.
